The Neurological Circuitry of OCD

OCD is a disorder of brain circuitry, not just "anxiety."

Research indicates that Obsessive-Compulsive Disorder is driven by hyper-connectivity and overactivity in a specific circuit in the brain known as the cortico-striato-thalamo-cortical (CSTC) circuit.

Within this loop, the anterior cingulate cortex (ACC) and the medial prefrontal cortex (mPFC) become stuck continually sending "error" or "danger" signals. The brain is essentially sounding a false alarm that cannot be easily shut off by logic, driving the individual to perform compulsions to temporarily relieve the distress.

Because these originating structures are located deeper in the brain than the areas responsible for depression, standard treatments often fail to reach them.

Brain mapping of OCD circuitry

Reaching the Root with Deep TMS

Specialized helmet coils designed to penetrate the cortex and reach the ACC.

Patient receiving Deep TMS

To treat OCD effectively, we utilize FDA-cleared Deep TMS (dTMS) technology. Unlike standard figure-8 TMS coils used for depression, dTMS uses a modified helmet design that projects the magnetic field deeper and broader into the anterior cingulate cortex and medial prefrontal cortex.

By delivering high-frequency magnetic pulses to these specific structures, dTMS regulates the abnormal activity within the CSTC loop, turning down the volume of the "false alarms" and diminishing the intense urge to perform compulsions.

01

Deep Penetration

Specialized H-coils reach areas standard TMS cannot, directly targeting the source of OCD.

02

Provocation

Treatments often briefly expose the patient to their trigger right before the session to activate the circuit so it can be effectively modulated.

03

Down-Regulation

Continuous sessions physically rewire the neural connections, reducing the intensity of obsessions.

OCD Symptoms Diminished by TMS

Effective across various OCD sub-types.

Intrusive Thoughts

Reducing the frequency, duration, and distress caused by unwanted, distressing obsession patterns.

Compulsive Urges

Dampening the overwhelming physical and mental drive to perform rituals (checking, washing, counting).

Pure-O Symptoms

Highly effective for patients whose compulsions are entirely mental (rumination, mental checking, reassurance seeking).

The Benefits of Non-Systemic Care

Why Choose TMS for Severe OCD?

A vital step for those stuck in the cycle despite medication and ERP therapy.

Avoids High-Dose Side Effects

Treating OCD biologically often requires massive doses of SSRIs, which carry significant side effects. TMS offers an alternative pathway that bypasses the gastrointestinal system entirely.

A Catalyst for ERP

Exposure and Response Prevention (ERP) is the gold standard therapy for OCD, but it can be too terrifying for severe cases. TMS turns the volume down, making ERP tolerable and effective.

FDA-Cleared Efficiency

As an FDA-cleared intervention specifically designed for OCD, insurance coverage is widely available for patients who have previously failed to find relief with SSRIs.

★★★★★
I've been dealing with depression and PTSD for years and Dr. Amin's TMS Therapy has honestly saved my life. I'm someone who does not respond well to depression medication, so TMS was a godsend for me. I went from crying for no reason almost every day to feeling more stable and even optimistic. Dr. Amin is really invested in my mental health and Charlie, the TMS technician, is very helpful in making the treatments go by quickly and swiftly. I don't even want to imagine what my life would have been like without these treatments these past few years.

I've been experiencing night terrors for a few years. I called Dr Amin immediately and he called me back right away. He told me he could help me. He came up with a medical plan with his diagnosis and that's what we're doing now. Dr. Amin is not aggressive and started very cautious with treatment because of my family history. Dr. Amin calls me to follow up on how I'm feeling. I highly recommend his practice. They answer in a timely manner. Everyone is very nice.

Dr. Amin saved me from my horrible OCD. Thank god for him. I originally went to him for a second opinion. I decided to take a leap of faith and start TMS after I had a bad OCD episode. It was rough in the beginning when I first started TMS but I persevered my way through and I'm so glad I did. I'm just about half way through and I see so many benefits already. I have my life back. I feel such relief — I can rationalize my thoughts and emotions so much better now. Overall I'm a better person. Thank you x1000 Dr. Amin!!!

I have been a patient with NJ Healthy Minds for the past couple of years, and choosing them has been one of the best decisions I've made for myself. Everyone I've encountered has been kind, compassionate, supportive, and consistent. The passion and care they show truly reflect their purpose and the positive impact they make in people's lives every day. I am not the same person I was when I first came to them. Their support has made a tremendous difference in my life, and for that I am deeply grateful. Dr Amin & staff, you rock.

Frequently Asked Questions

TMS for OCD FAQs

For OCD, TMS targets deeper brain structures, specifically the medial prefrontal cortex and anterior cingulate cortex, which are hyperactive circuits responsible for obsessive thoughts and compulsive urges.
Yes. OCD treatment often utilizes a specialized coil (like the H-coil or specific targeted positioning) to reach deeper neural networks compared to the standard focal coil used for depression.
While there is no "cure" for OCD, TMS is a powerful tool to significantly reduce the frequency and intensity of obsessions and compulsions, making them much more manageable in daily life.
Often, yes. Brief, customized exposure exercises (provocations) are performed just before the TMS session to activate the OCD circuitry, making the magnetic pulses much more effective.
